0166(3)(a) During the visit on 3/14/24, it was observed that the main exit door had a child doorknob cover on the interior. It was then removed during the visit and the rules for keeping the door unlocked from the inside were discussed. 0166(4)(c)(ii)(iii)(iv)(v)(vi)(vii) During the visit on 3/21/24, it was observed that the two exit windows from the back rooms measured 21.75 x 32 = 696 sq inches and not the required 720 (5 ft) of open area. Licensor will follow up about whether a waiver is possible.
